Current or Recent Patients of Millennium Physician Group (MPG), HarmonyCares, Mara’s Lighthouse Counseling Center, My Home Doctor, Universal Family care and/or North Florida Medical Center (NFMC):
Patients currently established with these organizations are encouraged to continue care with their existing providers to support continuity of care and avoid potential contractual conflicts. Consideration for new patient scheduling may be reviewed on a case-by-case basis if it has been more than five years since the patient’s last visit with one of these organizations.
